Adding ACC ICP Outcome Measures

This document covers entering and sending Outcome Measures for ACC ICP Contracts.

Adding PROM Measurement Types

1. Go to either
        a. Contracts >> All Contracts >> Search for the client's contract >> click the  icon
        b. Appointment and select the   button
2. Click Add PROM

1. Select Measurement Type from the drop down box.  Note: This list is provided by ACC and cannot be edited.
2. Enter the Measurement Date
3. Select the Stage . Note: This list is provided by ACC and cannot be edited.
4. Enter the Measurement Value.  Note: If unable to provide data, select the options in Data Absent Reason.
5. Click >> Send To ACC

1. When the Case Sent Successfully pop up appears, click >> Ok . Note: If prompted Allow permission for the Digital Certificate.

2. When returned to the  ICP Outcome Measurement window click >> Save

1. The Outcome Measurements table will be updated on the ICP Outcomes tab

Adding Clinical Measurement Types

1. Go to either
        a. Contracts >> All Contracts >> Search for the client's contract >> click the  icon
        b. Appointment and select the   button
2. Click Add Clinical Measurement

1. Select Measurement Type from the drop down box.  Note: This list is provided by ACC and cannot be edited.
2. Enter the Measurement Date
3. Select the Stage.   Note: This list is provided by ACC and cannot be edited.
4. Enter the Measurement Value .Note: If unable to provide data, select the options in Data Absent Reason.
5. Click >> Send To ACC

2. When returned to the  ICP Outcome Measurement window click >> Save

Reviewing Existing Outcome Measures

1. Go to either
        a. Contracts >> All Contracts >> Search for the client's contract >> click the  icon
        b. Appointment and select the   button

2. Outcome Measurements can be filtered by All, PROM, Clinical Measures and Measurement Type
